In this Adobe Photoshop tutorial you will see how to add a cool open wound with intestines to an image.

1.
First of we will start making the basic shape of the wound.
Make an empty layer in the layers panel.
With the elliptical selection tool drag out as shown i the image below.

Hold down shift and hit backspace, fill the selection with black.

Go to filters -> distort -> ripple and set it as shown below.

Now hold down ctrl and click the shape layer in the layers panel to load the selection, now delete the shape layer.
Go to select -> modify -> smooth and smooth it about 4 px to soften the edges.
Make a new empty layer and fill the selection with black.

Now we are ready to place the shape on to our model, so open up an image where you want to apply this wound to, and drag the layer on to the image.

Now i want to bend the shape so it follows the stomach shape.
Go to filters -> distort -> twirl and twirl it a bit as I did below.

7.
Now we will apply some styles to our wound.
So double click the wound layer to get to the styles panel and set it as shown below.

Now you should have a result similar to mine.
